What is a Positive Behavioural Support Plan?
It is an individualised care plan that clearly outlines the challenges being faced and what strategies, tools and supports TLC can provide to overcome them.
Positive Behavioural Support Plans ensures the entire support team within TLC, NDIS and other service providers have a clear understanding of everyone’s goals and support requirements.
TLC’s process of developing a Positive Behavioural support plan can include the following components:
- An initial interview with each person to understand what challenges they are facing as well as their goals
- We collaborate closely with the broader support network and stakeholders to understand what behaviours may be causing challenges
- If there are multiple service providers involved, we ensure there is a clear line of communication to ensure the support plan is accessible and relevant for everyone
- With all this information, we outline the strategies that best respond to each person's individual needs
- It is important that we address any restrictive practices currently in place and how we can reduce or eliminate them
